You will be a pivotal part of the Integrated Health and Social Care team to provide multidisciplinary working, that will also enhance care delivery to the patients. Having previous experience in a leadership role would be desirable.

The Community Nursing Team is skill mixed with registrants / non-registrants; you will be expected to share up to date learning with your peers, supporting and developing the team to deliver excellence.

You will be provided with regular supervision and support with ongoing professional development as identified in your annual appraisal.

This Band 6 Community Nurse WTE month fixed term post is the perfect opportunity for continued learning and career progression. If this ignited your interest we would like to hear from you.

To be a District Nurse/Community Sister/Charge Nurse your role is vital in terms of coordinating peoples care and delegating to your team. Develop knowledge of government policy and clinical procedures.

You will be required to be an autonomous practitioner, a role model to provide holistic patient centred care. To champion for patients, whilst demonstrating highly motivated leadership skills with ability to influence and inspire others. Teach and mentor your team utilising your transferable skills.

The successful candidate will need to ensure that high standards of Nursing Care are delivered to patients on the team caseload and be responsible for addressing any areas of poor performance and identifying and promoting service improvement. You will be encouraged to engage in decision making daily.

Manage sensitive issues and overcome obstacles in communication.

We you be required to undertake a robust induction, allocated a supervisor to support your learning to develop in your role.

We offer training in line with the required competencies for you to build on your past knowledge, experience, and skills. You will be expected to undertake continued learning with accessibility to accredited courses.

You will be required to be competent, diagnose and investigate. Have sound knowledge and judgement providing evidence-based wound management, pressure ulcer prevention, palliative, and end of life care.

Have a full driving licence and the ability to travel within the Culm Valley.

Working Pattern: hours per week, Monday to Friday 9: - :, incorporating weekends and bank holidays
