Job summary

You will have a backgroundin working with people living with long-term conditions or cancer, ornutrition, exercise, health improvement, adult and/or family weight management,smoking cessation or similar and be an excellent team player and have awillingness to learn and develop.

Main duties of the job

ABLHealth will soon be launching a new Healthy Lifestyle service inGloucestershire, commissioned by Gloucestershire County Council. Thistransformative service seeks to work collaboratively with communities totransform the lives of the local population. The service will support residentsto help them lose weight, stop smoking, reduce alcohol consumption, get moreactive and improve their wellbeing - all embedded in sustainable behaviourchange. The service will also deliverthe 1001 days programme supporting families and young children and the HealthyWorkplace award scheme.

Job responsibilities

Duties and Responsibilities

Deliver ABL Healths Healthy Lifestyles service, through one-to-ones, in the community, and through digital methods such as Zoom/Teams or phone appointments. Deliver evidence-based education around nutrition, physical activity and behavioural change as advised by ABL Health team and in line with ABL Health programme curriculums. Deliver programmes with a client-centred approach using skills of positive psychology and motivational interviewing. Support with delivery of 1001 day or workplace programme as required. Monitor client's progress through regular consultation and with the use of Microsoft excel, identifying specific treatments for those not on track to achieve their goals. Work with clients and stakeholders to co-produce in-session materials. Contribute to the contract performance report by submitting regular case studies to the project lead. Contribute to and support ABL Healths social media pages. Contribute to training and support new staff and volunteers. Working with individuals and groups using SMART goal-setting. Keep detailed files, recording data accurately, ensuring all forms and other documentation is completed correctly. Have a thorough understanding of ABL Health policies and procedures. Engage and recruit potential clients at promotional and community events. Contribute to ongoing service development needs. Build community links and partnerships with long-term condition and/or cancer leads to encourage new referrals into the service and maintain close working relationships with other community services. Present service and referral pathway information at stakeholder meetings as directed by the Head of Service To attend and actively participate in team and wider company meetings, such as MDT. Be able to organise own workload in order of priority and deadline dates, daily administration tasks and other work-related activities. To demonstrate personal and professional development in line with the ethos of lifelong learning To work within the framework of clinical governance to maintain and improve patient care, quality and efficiency. Qualifications

Essential

Educated to degree level or relevant equivalent experience.. Registration with CIMSPA (this can be obtained within 6 months of employment).

Desirable

Sports Coaching qualification Degree in nutrition and/or eligible to be registered as an associate or full member of the UK Voluntary Register of Nutritionists/ or degree OR in exercise, psychology, public health, health improvement other health related field.

Experience

Essential

Experience of working with Adults with long term conditions delivering group workshops in an age-appropriate and engaging manner Experience of delivering groups and/or one-to-one behaviour change Experience of delivering health promotion / public health interventions in community settings Experience working with hard to reach groups and tackling health inequalities. Experience of working within a multi-disciplinary team. Previous paid or voluntary experience working with people. Experience of building professional relationships

Desirable

Experience of delivering groups and/or one-to-one specialist weight management, Smoking Cessation or similar interventions to children, young people and adults. Experience of working within a multi-disciplinary team. Experience of planning and delivering appropriate health interventions specific to individual requirements. Experience of working with people with long-term conditions or cancer.

Job Specific Requirements

Essential

Excellent written and verbal communication skills, with the ability to build rapport quickly with people. Excellent administration, record keeping and data collection skills. IT literate, preferably with experience using Microsoft Office (MS Word, Excel, Outlook). Excellent time management and organisational skills Must have a high attention to detail. Pro-active approach and ability to work on own initiative. Ability to manage stressful situations and cope in a fast-paced work environment. To cope with unpredictable work patterns and work well under pressure. To be able to work as part of a team and independently as required.

Desirable

Knowledge of safeguarding. Experience of delivering parenting programmes. Experience of contributing to the supervision of staff/ volunteers/ students. Experience of developing public health educational materials. An understanding of different social media platforms. An understanding of how social media can support positive behaviour change.

Essential

The ability to travel independently across Gloucestershire. Work flexibly and adapt to suit service need including evening and weekend work as required. Must be willing to undertake an Enhanced DBS check.
