Collaborate within multidisciplinary teams, utilizing clinical judgment and autonomous decision-making. Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions, and omissions at this level of practice. Demonstrate a critical understanding of their broadened level of responsibility and autonomy, recognizing the limits of their own competence and professional scope of practice, especially when dealing with complexity, risk, uncertainty, and incomplete information.

Initiate, evaluate, and modify a range of interventions, which may include prescribing medicines, therapies, lifestyle advice, and care. Exercise professional judgment to manage risks effectively in complex and unpredictable. Work collaboratively and carryout joint visits members with MDT colleagues within the team to promote holistic person-centered assessment and approach to care. To continually have awareness of the need to respect people’s privacy, dignity and individuality and aim to provide care in an environment which is appropriate to the wishes of the person and to their current physical and emotional needs. To continually develop and review acute illness pathways and practices to ensure that services are delivered efficiently and effectively providing best value to people and organisations. Evidence of relevant involvement in meeting the Trust clinical governance objective.
