Job summary

DHC is offering a great opportunity to bepart of providing really high quality and innovative care to frail patients inthe Dorking area.

As a Frailty Care Coordinator, thesuccessful applicant will work closely with clinical teams helping to provideoptimal care for frail patients. Therole involves some patient contact by telephone and in person, and a positive, respectful, and compassionatenature is necessary when delivering a high-quality, safe and effective servicewith the highest standards of confidentiality.

The Care Coordinator will be provided withall necessary training to fulfil the project to its full potential.

Main duties of the job

DHCis looking for a motivated and compassionate person who either has, or isworking towards, a healthcare certificate,and who enjoys patient contactand the challenge of providing great quality care. The role involves some homevisits and some clinical care, and you will work with a variety of clinical andnon clinical staff to ensure that frail patients receive the most joined up andappropriate care possible.

You will use data searches to seek out patientsfor care, and support with innovating pathways for frail patients, along with ateam of healthcare professionals. You will also liaise with care homes in thearea to optimize the relationship with primary care for patient benefit.

Job description

Job responsibilities

The Frailty Care Co-ordinator will have a pivotaladministrative role in working with the practices and the widerprimary/community care team to ensure frail patients receive appropriateinterventions to support them to live as independently as possible. This willinvolve supporting the co-ordination of a range of clinicians and socialcare. It will also involve working withindividual patients to identify, then facilitate any support they would benefitfrom being delivered by the voluntary sector or health services.
