Job summary

An exciting opportunity has arisen for a Band 7 Diabetes Specialist Dietitian who is passionate about delivering high quality care, motivated and demonstrates the Vision of our Trust within our Community diabetes team

The post holder will work across community settings.

You will be an enthusiastic and experienced dietitian with a warm personality and great people skills. You will need to be adaptable to change and be able to influence new ways of working.

You will be responsible for providing high quality evidence-based care for people with diabetes, their families, and carers.

Work as an integral member of the Diabetes team.Support the provision of specialist Diabetes dietetic knowledge, advice and education to other staff, the wider multi-professional team and the patient and their carers.Support the provision of education and training for nursing, allied health professionals and other health care professionals to promote insight into the specific needs of adults, including health promotion.

Main duties of the job

To deliver a high-quality patient centred care and education to enable people with diabetes to manage their condition effectively.

To provide the highest standard of care for all types of diabetes patients in collaboration with the multi-disciplinary team to improve and maintain the quality and safety of patient care.

To contribute to the delivery and development of the Diabetes service using effective evidence-based practice in line with agreed national standards for clinical practice.

To continuously improve clinical care standards in line with the clinical governance framework to ensure the highest quality of care is provided in safest manner.

To provide specialist advice and support to clinical staff within Oldham Care Organisation on the management and care of patients within their own area of specialist practice and to facilitate the effective delivery of care to patients.

To always act as a resource and role model for the profession.

To provide education and training to clinical staff and internal /external agencies.

Support GP surgeries and Primary Care Networks in their management of people with diabetes to reduce variation in Primary Care through practice support and education.

Job description

Job responsibilities

Working within the community setting you will lead the dietetic provision to the multidisciplinary diabetes service. You will provide education to patients with diabetes who have complex dietary caseload will include MDT clinics as well as dietitian led clinics and group education sessions and home visits. You will provide dietetic intervention, to support patients in the areas of carbohydrate counting, insulin dose adjustment, flash/continuous glucose monitoring, carbohydrate will represent the diabetes service at relevant internal and external meetings. You will be actively involved. in training dietetic students, and you will also provide training for a variety of health professionals whilst being a source of support to all colleagues around Diabetes care.

Essential

Bsc/Msc in Nutrition and Dietetics or postgraduate diploma in Nutrition and Dietetics Several years clinical experience following graduation Experience of working with patients with diabetes including T1DM, T2DM and those with other pancreatic disorders Excellent knowledge of Nutrition and Dietetics and an understanding of the dietary management of diabetes

Desirable

BDA course in specific area of practice Non medical prescriber V300 Accredited Desmond educator Accredited diabetes course
