This is an exciting time to join our Adult Community Nursing Service, as we continue to develop in response to the changing needs of communities, and in line with the NHS Long-Term Plan. We are currently working with our partners towards improving both the delivery and access to our services including the development of Integrated Neighbourhood Teams.

You will be required to bring evidence of your qualifications when attending your face to face interview.

Please note, this role also requires you to have a Driving License.

Main duties of the job

Adult Community Services has an excellent Practice Education Team who will support your development, enabling you to provide excellent care to a diverse population of patients. You will be supported with a full induction and as a Trust we offer excellent flexibility, and we can discuss your shift pattern at interview.

Main duties of the job includes, working as part of the Integrated Neighbourhood team, you will have the opportunities to develop or extend your knowledge and experience of caring for patients in their own home by working across a range of clinical settings. There are a list of core and additional clinical skills that are required for the role which is included in the attached job description.

Job description

Job responsibilities

Working under the direction of a registrant, participate fully in the delivery of planned nursing care, within the boundaries of your role which may include chaperoning of patients, escorting/ transferring of patients and providing 1-1 patient centered care or caring for a group of patients that require enhanced care.

Demonstrates own duties to new starters and supervises work of students and HealthCare Support Workers

Frequent exposure to emotional/distressing events, , breaking of bad news.

Assist in the care of patients at end of life and after death.

Orders supplies under the direction of the nurse in charge.

Participate in patient and public involvement activities.

Participate in general cleanliness and tidiness of ward environment.

Undertake mandatory training and role specific training relevant to role.

Record and escalate patient physiological observations.

Demonstrate an understanding of sepsis and when to escalate to a Registrant/ Medic

Acts on own initiative when delivering patient care with supervision accessible.

Undertakes a range of core skills which include:

Venepuncture Patient observations including Blood pressure, Oxygen saturations, heart rate, respirations, temperature, pain score, News score Peripheral Cannula removal Swabbing to include MRSA, FLU, COVID, Obtain samples including urine dip, from a catheter, MSU and faeces, sputum, Blood glucose and ketone testing ECG Observe wounds and change simple dressings Bladder scan Skin assessment Catheter removal Female catheter insertion Pregnancy testing

There may also be role specific clinical skills that you will be required to develop competency in to undertake your role (see Addendum One).

Addendum One - Additional Role Specific skills include:

Assisting with complex wounds Removal of stitches, clips and sutures 3-layer compression (urgo. Actico dressings) Medication eye drops, insulin injection pens, topical, tablets, B12 injections, anticoagulation injections Support the rehabilitation of patients Doppler (planned care only) Patient education End of life care Person Specification

Qualifications

Essential

Level 2 in numeracy and literacy (or equivalent) Completed Care Certificate and have evidence of both the theory and practical assessment completed. Level 3 health-related Certificate/Diploma from a regulated education body or, for existing Mid Yorkshire staff evidence of Band 2 competency booklet completed. Any new employees who can evidence competency of core skills of the Band 3 Job description.

Any further qualifications and training relevant to this post.

Ability to learn new skills and adapt existing knowledge. Understanding of responsibility, accountability and confidentiality, relevant to the healthcare role To have a basic understanding of the NHS and the roles of nurses, therapists, and social care professionals within a healthcare setting. Knowledge of care and related procedures and relevant legislation. Ability to demonstrate knowledge of data protection and health and safety issues.
