This is an exciting opportunity to be at the forefront of the deliveryof our new Ageing Well model of care Team Up Derbyshire. The role transcends the traditional healthand social care organisational boundaries.It will involve true MDT working, building relationships with GPpractices, Community Rapid Response services and Falls services, to helpclinically deliver the service on a day-to-day basis.

You will be partof a team that leads on medical models of care across primary andcommunity-based settings, in collaboration with multi-professional andmulti-organisational colleagues. You will also play a crucial role in step up,step down and anticipatory care through timely assessment, referral, responsiveintervention, and care planning.

Main duties of the job

Work as part of Team Ups MDT to provide and monitor care, under direct or indirect supervision.

Implement and evaluate individual care plans, utilising practical and theoretical knowledge under the direction of a registered practitioner.

To work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients and where appropriate, refer people back to other health professionals within the PCN; Be responsible for co-ordinating all of the elements involved in running a successful MDT meeting Co carry out environmental assessments and consider need for equipment, including onward referral to appropriate MDT members and ensuring safe use. Improve safety and quality of care at every opportunity.

Key duties and responsibilities (Clinical)

Work as part of Team Ups MDT to provide and monitor care, under direct or indirect supervision. Improve safety and quality of care at every opportunity. Perform area specific clinical responsibilities such as:

o Monitoring of physical observations

o Venepuncture

o Long Term Condition Monitoring

o Blood Glucose Monitoring

o Administration of treatment , nebulisers, oral medication.

o Diabetic Foot Checks

Use appropriate risk assessments, such as NEWS2, in the assessment of patients, to identify patients at risk of deterioration of those that are acutely unwell and require further intervention, , sepsis. Undertake assessments and reviews, and care for individuals with dementia, mental health conditions, and learning disabilities. Ability to makes judgements which require assessment of facts, , escalating abnormal clinical observations, contraindications, emergency situations, wound care, and deciding on implementation of care programmes where there are a number of options. Ensure appropriate escalation to maintain patient safety at all times. On the direction of a registered practitioner, refer patients to relevant professionals for specific treatment, advice or support. Act as a role model by upholding and implementing good practice in the workplace, always ensuring the highest standards of evidence-based care. Ensure correct procedures are followed in the event of an incident or accident involving patients, families or members of staff. Demonstrate competency of clinical skills evidenced within policies and procedures. Participate in clinical supervision. Recognise own responsibilities within delegated scope of practice and to refer to others when beyond own scope or competency. Provide support and supervision to training nursing associates, healthcare assistants, students, apprentices, and those on learning assignments/placements as required including delegation of tasks suitable to their role. Undertake daily risk assessment of clinical situations and completion of incident reports as necessary in accordance with policies. Record all interventions and maintain accurate and contemporaneous patient health records in a timely manner.

KEY DUTIES AND RESPONSIBILITIES (Other)

Participate in quality improvements and innovations, , audits, significant events analysis and development of protocols and new services; To offer appropriate support and guidance to patients and their families / carers, utilising decision making aids in preparation for a shared decision-making conversation; Holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person; To provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers and other primary care professionals; To undertake general administrative duties to support the role and any other reasonable duties as requested by a manager to ensure quality of service Support the work of the registered practitioners in the Team Up team to enable them to focus on the more complex clinical care. Support the delivery of services to enhance care to care homes, the housebound and frailty population through virtual, telephone, face to face and home visit reviews. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services. Exhibit and maintain a high personal standard of expertise, competence, attitude and behaviour at all times, observing the rules of confidentiality and respecting the patients rights. Develop relationships across the PCN to support integration of the role across health and social care including primary care, secondary care, and mental health, and contribute to the delivery of integrated care. Maintain excellent communication with patients, relatives, care/residential home staff, and members of the team demonstrating a variety in communication skills in accordance with patient needs. Adapt communication style to meet the needs of the individual and breakdown any existing barriers to communication therefore using appropriate means to convey messages and support the expression of thoughts and ideas. Communicate proactively and effectively with all Team Up MDT colleagues, attending and contributing to meetings as required. Promote health and well-being to all patients, for example undertaking the NHS health checks. Contribute to public health campaigns (, COVID-19 or flu clinics) through advice or direct care. Ability to plan and organise own workload for home visits and audits. Chaperone patients when required. Contribute to and embrace the spectrum of clinical governance. Maintain responsibility for the identification of continuing educational needs and development and maintain appropriate levels of competency under the supervision of the clinicians. Actively participate in the annual appraisal process and formulate a personal development plan. Undertake all mandatory training and induction programs, and any relevant courses associated with the role.
