Job summary

The role is for a General Practice Assistant, Clinical Administrator, supporting two practices within our PCN, based in South Northants.

. This is a 12-month fixed term contract, for maternity cover, part time, 30 hours per week.

It is essential that you own and drive a vehicle as these are rural practices and you will be required to commute between the two sites.

You will need to possess strong organisational skills and be able to communicate with a variety of people, including patients, clinicians and other NHS colleagues.

You will need to demonstrate resilience and adapting to change, in the event of duties developing or altering to meet patient or business needs. You will be self-motivated and not afraid to seek answers and find solutions.

You will be able to professionally respond to patient complaints, demonstrating empathy and understanding whilst outlining practice policies. You will possess good knowledge of Excel and Word and have exceptional admin and coordination skills.

You will be personable and need to develop working relationships.

A knowledge of the SystmOne clinical system is essential, as working from this system will form the basis of your daily tasks.

You will work closely with the Practice Manager at each site with regard to your daily tasks / duties in line with practice protocols and objectives, and to ensure you are delivering against these.

You will report into the PCN Operations Manager.

You will also be expected to participate in PCN meetings / training.

Main duties of the job

Your duties will be to support the practices around all clinical administrative tasks, ensuring patient records are up to date and coded correctly. You will also be involved with patient referral admin. You will be required to complete other administrative duties that the practice managers deem appropriate.

Job responsibilities

The Successful Candidate will be required to:

Maintain accurate and update patients records, recording data consistently and according to practice protocols Pro-actively participate in the gathering and recording of health information Accurately read coding patient data on SystmOne from letters, discharge summaries, A&E reports etc. Review progress of summarising, ensuring practice targets are achieved Scan patient related documentation and attach scanned documents to patients healthcare records from post, electronic letters and emails Input data into patients healthcare records as necessary Run reports as required, for either patient specific information or patient cohort information Responsibility for the Choose and Book referral system for both internally and externally, ensuring patients are communicated effectively and follow up letters are sent where appropriate Ensure practice policies are followed and accurate records kept, with particular reference to referrals, matching and coding of patient correspondence. Action tasks and emails sent via the clinical systems and NHS email Ensuring all referral forms are completed correctly (with mandatory information, if required) & submitted via eRS (Electronic Referral Service) or email, to include Prior approval and IFR, Advice and Guidance, Hot Clinic referrals and Expedite referrals as required. Complete referral audits, especially 2ww referrals quarterly Document patient record with ERS referral outcome and advise clinicians where appropriate Assisting patients and colleagues with information about referrals Daily checking of eRS worklists and dealing with queries etc appropriately Liaising with external providers which may include chasing up referral information, querying possible errors, asking for advice and pathology results Ensuring that all 2WW (Two Week Wait cancer) referrals are sent via the fast-track Referral system and have been allocated an appointment (checking task list against Enquiries tab on eRS Enquiry) and read coded, and patient sent a new diagnosis letter Person Specification
