Job summary
We are looking to recruit an experienced and enthusiastic healthcare assistant, to provide holistic individualised care and support to children and young people with life-shortening conditions and their families across West Sussex and South-East Hampshire.
Main duties of the job
Caseloadmanage, including key working, providing short fun breaks in the community andhome setting, prioritising workload, escalating concerns to staff nurses
Under supervision, participate in the delivery of high quality,care to children and young people who receive short breaks and end of life carefrom Chestnut Tree hospice services.
Work within other areas of the Hospice as directed to do so to maintain service continuity and responsiveness in-house team.
Provide clinical care within a competency framework of delegatedduties Identify and report on significant changes inchildren and young peoples conditions or wellbeing outside own areas ofcompetence.
Work within own scope of competence andto be accountable for a delegated task.
Implement care plans, as delegated by Registered Nurses to meetthe individual needs of each child/young person, upholding their dailyroutines, as far as is practical.

Job responsibilities
To assist in providingspecialist Children / Young Peoples (CYPs) palliative care including respiteprovision and end of life care as part of a multi-disciplinary team in childrenand young peoples homes.
To work within anintegrated care team, working within other areas of the Hospice as directed todo so in maintaining service continuity and responsiveness inhouse team
To provide personal andclinical care to meet the needs of children and young people as well ascreating and providing opportunities for children and young people toexperience play, social interaction and learning.
