Job summary

This is a new, exciting, and innovative post to support the needs of the local population living with mild to moderate mental health difficulties. We are looking for a registered nurse, occupational therapist, or social worker to take a central role in the development of the mental health offer in our Primary Care Network (PCN). Through the delivery of excellent, compassionate services to promote the recovery and well-being of patients, carers and families.

At Waterside PCN we support patients attending Forestside Medical Practice, Red and Green Practice and Waterfront and Solent Surgery. We have a population of 41,854, we see many people of working age in need of support to aid their mental health recovery. At Waterside PCN we can offer flexible working, including job shares, later start and finish times and some Hybrid working.

You will be joining a well-established, friendly, and committed team who prioritise patient centred practice. We have a community garden which is run by patients and volunteers and was set up to offer connection for the community. We continue to explore innovative options to meet the patient demand. Our PCN is in a beautiful part of the country which is a pleasure to work in and aids work life balance, as there is the opportunity to take breaks in a wonderful setting.

Main duties of the job

You will work in a multi-agency way supporting the interface between Primary Care and Secondary Mental Health Services, to provide specialist advice and support, assessment, treatment, education and solution-focused approaches to the local Primary Care Teams.

You will be professionally accountable and responsible for patient care.

Triaging and undertaking initial assessment, signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in GP surgeries, patient's homes and other primary care settings.

Providing clinical advice to GPs such as specialist opinion, this will contribute to the safe and effective day to day operation of the PCN and the efficient and timely management of clinical enquiries and decision-making.

Maintain clinical knowledge and skills by attending training and applying this in practice to support the development of the team.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

Qualifications

Essential

Qualified as a registered mental health nurse, social worker or occupational therapist Mentorship course / evidence of CPD Ability to fulfil travel requirements of the post Clinical supervision training More than 2 years' experience of working with clients with severe and enduring mental health needs
