The ward has17 beds and a palliative care suite and provides sub-acute and palliative careas well as complex rehabilitation. The ward is one of thirteen which form theTrusts Inpatient Community Health Services Division and the successfulcandidate will play a vital role in providing compassionate patient centredcare. Our cohorts of patients are increasingly complex due to the need tofacilitate transfer from acute trusts for individuals who are medically fit fordischarge and to prevent admission from home to the secondary care sector.

Main duties of the job

Assessing,planning, implementing and evaluating patient care in a nurse led environment.

Assisting withand contributing to the management and organisation of the ward.

Delivery of high-qualitycare.

Participatingin education, development and supervision of other staff members and students.

Workingcollaboratively and co-operatively with others to meet the needs of clients/ service users and their families.

Qualifications

Essential

Level 1 Registered Nurse (Adult). Student Nurse in the final year of an adult nursing programme.

Experience

Essential

Demonstrates effective communication and interpersonal skills. Demonstrates adaptability and flexibility dependent upon work priorities. Demonstrates an understanding of current developments in the NHS and their implications for nursing care. Demonstrates an interest in providing patient centred nursing care and patient education.

Standard Requirements

Essential

Demonstrates an understanding of equality and valuing diversity principles. Demonstrates an understanding of confidentiality and data protection. Demonstrates an understanding of the needs of service users of the Trust.
