We are seeking a dynamic, highly motivated individual who is committed to providing a high-quality service.
This a fixed term or secondment opportunity with a focus on strengthening pathways between Merton and Sutton Mental Health Learning Disability Team and other primary care and secondary care services. With a focus on ensuring individuals are supported appropriately on discharge from the team, the post holder will have an active role in developing relationships with borough based services. The post holder will work alongside colleagues in the team to develop person centred care plans which focus on the person's mental, physical and emotional wellbeing.
The Sutton and Merton Community Mental Health Learning Disability Team provides a service to adults with a learning disability who are experiencing mental health problems and have needs that can not be met by mainstream mental health services.
You will be part of a small and friendly multidisciplinary team working with colleagues who are experts in the field and are dedicated to improving health outcomes for individuals with learning disabilities. The multidisciplinary team includes Psychiatrists, Nurses, Psychologists and a Recovery Support Worker. Care co-ordination is provided to a small number of service users.
Main duties of the job
To work as an autonomous, highly specialist practitioner, managing a client caseload as agreed with the Team Manager
To lead in care pathway development and management with a focus on improving referral and discharge pathways between SWLStG Community Learning Disability Teams and primary care and other providers of learning disability care (such as Community Teams for People with Learning Disabilities in Local Authority)
To provide advice and expertise in care planning and risk management to other health and social care professionals
To provide support, supervision and education to carers and clients with complex needs
To participate in the teaching and development of colleagues, students and colleagues in other agencies about this specialist area
To take responsibility for being the lead HCP in identified clinical areas, as delegated by the Team Leader
Act as team deputy and provide cover for the Team Leader as required
To contribute to and/or lead on service development initiatives and projects
To initiate and participate in audit/research projects associated with the work of the service when required.

Job description
Job responsibilities
To be responsible for carrying out robust specialist assessments of those referred to the service. To be responsible for making clear autonomous decisions based on an analysis of complex presenting problems and judgement about available options.
To utilise a wide variety of clinical skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen.
To work with colleagues to develop and promote the services philosophy, framework of care delivery and strategic objectives, taking responsibility for specific areas of policy and strategy delivery.
To work with colleagues to develop clinical expertise and high standards of nursing practice, as defined by the Nursing and Midwifery code of professional conduct
To deliver care that is evidence based, reflecting current best practice and research.
To participate in structured clinical supervision in accordance with Trust policies and procedures and to take responsibility for the supervision of nurses.
To demonstrate clinical excellence and high standards of nursing practice to other staff.
To demonstrate sound understanding of the legal and ethical issues in caring for people with mental health issues and learning disabilities
To provide clients and carers with verbal and written up to date information on the services offered by the Trust, how to express their views, complaints procedure and what they can expect from the service.
To provide highly specialist knowledge and expert advice to health and social care professionals providing specialist support and intervention to patients, families and carers.
To communicate clearly and sensitively in situations that are contentious and/or hostile, demonstrating a high level of interpersonal skills, self awareness and empathy in all communications.
To contribute to weekly MDT meetings to discuss referrals, treatment options and action plans.

Training & Qualifications
Essential
RMN/RLDN with current NMC registration ENB 998 or equivalent Evidence of continuing professional and personal development
Desirable
Educated to degree or diploma level nursing or other courses relevant to service Nurse prescriber

Demonstrable experience of assessing people with learning disabilities and mental health needs Experience of working with people who have experienced serious ongoing mental health problems Experience of working in learning disabilities services Experience of assessing and managing crisis Experience of working as a Clinical Supervisor Experience of working within a multidisciplinary team Experience of supervising junior staff within own and/or other disciplines
