The Nelson Medical Practice in Wimbledon Chase has a new and exciting opportunity for a trained Nurse Associate to join our friendly and professional team. We are dynamic and forward thinking in our approach to patient care, and we enjoy the facilities of working in a modern purpose built health centre and we pride ourselves on delivering high quality patient care. We are looking for a Nurse Associate to join our Nursing Team where they will be working with the support of our team of experienced Practice Nurses as well as integrating fully with the wider clinical team which includes a whole range of allied health care professionals. The successful candidate will participate within range of practice Nursing activities from chronic disease management, lifestyle advice,delivery of immunisations, wound care management and administering B12 successful candidate will have regular meetings with a clinical mentor to support their success and to help with future career development.
Main duties of the job
The applicant must be a Registered Nurse Associate on the NMC register and must have the Nursing Associate foundation degree qualification.
The main duties of the role are:
Delivery of high quality, compassionate care whilst undertaking specific clinical care tasks under the direction of a registered nurse, with a focus on promoting good health and independence.
Be actively involved in the provision of health education and self management advice to patients, relatives and careers.
Support individuals with all aspects of care including daily living, providing person centered care and promoting health and independence through raising awareness and care navigation to other healthcare professionals as appropriate.
Work with a supervisor to take responsibility for developing own clinical competence, leadership and reflective practice skills within the workplace.
Performing and recording clinical observations such as Blood pressure, temperature, respirations and pulse.
Discussing and sharing information with registered nurses about patients health conditions, activities and responses.
Developing and understanding of caring and supporting people with Dementia, mental health conditions and learning needs.
Carry out NHS Health checks
Carry out urinalysis of urine samples for in house testing in accordance with practice protocols.

Our team consists of Partners, an Executive Management Team, Salaried GP's, an experienced Nursing Team that is well led, Clinical Pharmacists,Health& Wellbeing Coaches,a First contact Physio, Paramedics, Physicians Associate as well as an experienced team of support staff. We are also a training Practice, training GP's medical students and Nurse Associates. We also provide mentoring.
Even though we are a large team we are very sociable. We have a Summer BBQ, Christmas Party, Practice buffet lunch every other month, Practice breakfast every Monday morning and other team events throughout the year.
Job description
Job responsibilities
Delivery of high quality, compassionate care whilst undertaking specific clinical and care tasks under the direction of a registered nurse (or other registered care professional), with a focus on promoting good health and independence.
Be actively involved in the provision of health education and self-management advice to patients, relatives and carers.
Support individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate.
Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.
Work as part of a PCNs multidisciplinary team (MDT), delivering a high standard of care that focuses on the direct needs of the patient.
Work with a supervisor to take responsibility for developing own clinical competence, leadership, and reflective practice skills within the workplace
Develop the skills and knowledge to provide direct care to patients and families which may include: after undertaking additional training, provide flu vaccinations, ECGs, venepuncture, and other relevant clinical tasks as required by the PCN, in line with the competencies of the role.
Supporting individuals and their families and carers when faced with
Un-welcome news and life-changing diagnoses, for example by providing relevant information on the diagnosis, signposting patients to further information, or referral to social prescribing link workers etc.
Performing and recording clinical observations such as blood pressure, temperature, respirations, and pulse;
Discussing and sharing information with registered nurses on patients health conditions, activities, and responses.
Developing an understanding of caring and supporting people with
Dementia, mental health conditions, and learning disabilities.
Assist in the implementation of appropriate interventions to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers. Assist in the assessment of and contribute to the management of risk within the environment where care is being administered. Carry out NHS Health Checks and clinical observations including, blood pressure, temperature, respirations, pulse and cholesterol testing. Measure and record patients height, weight, and BMI. Carry out urinalysis of urine samples for in house testing in accordance with practice protocols. Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being. Give lifestyle advice to patients (including diet, weight and exercise) making any necessary referrals to local support services as required. Give smoking cessation advice and initiate smoking cessation therapy. Assist in providing minor-surgery sessions, coil fits and implant fittings. Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with chronic diseases, CHD, Diabetes and Hypertension. Carry out cardiology diagnostic testing for internal and external patients. Perform spirometry tests for patients in line with practice protocols, ensuring that a PSD is used for the Salbutamol nebuliser. Meet the needs of patients presenting for non-complex wound care and use dressing formulary. Carry out stitch/ suture removal in accordance with practice protocol. Take venous blood samples from patients in line with practice protocols and local hospital pathology protocols. Promote health and prevent ill health by understanding the evidence base for immunisation, vaccination and herd immunity. Document and administer influenza and pneumococcal vaccinations according to a PSD. Document and administer B12 injections according to a PSD. Undertake clinical tasks including cannulation, venepuncture, ECGs. Accurately record nutritional and fluid intake. Ensure the privacy, dignity and safety of individuals is maintained at all times. Develop understanding of caring for individuals with particular conditions for example dementia, mental illness, learning disabilities. Recognise patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health. Demonstrate good understanding of the Mental Capacity Act and Deprivation of Liberties, applying principles to everyday practice or seeking advice from the registered care professional as required. Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals. Person Specification
