Job summary

The Continuing Care Team in Carmarthenshire are looking for an experienced HCSW to join our team. We provide care to both long term patients and specialise in end of life care at home.

Main duties of the job

The post holder will be required to deliver a high standard of person centred care in accordance with Care and Treatment Plans as directed by the registered nurse. Excellent interpersonal and communication skills are essential as is the ability to work autonomously and with colleagues.

The contractual base will be Carmarthen however the post holder will be required to work across the county. The service operates 7 days a week between 0800 - 0000 and staff shifts are within these hours. You will be working in patients' homes across the county and the ability to travel to locations in a timely manner is essential.

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
