Bank Clinical Pharmacist - Band 7
We are looking for enthusiastic pharmacists to join our growing bank pharmacy team for working anything up to hrs a week. Your base will be one of the 2 main sites- the main ones being Aintree or Royal Liverpool Hospital
Main duties of the job include providing a clinical pharmacist service to nominated wards within Liverpool Univeristy Hospitals NHS Foundation Trust (LUHFT). This will require working across a range ofspecialities including Emergency care, medical and surgical wards, taking medication histories and reviewing mediciation to optimise patient care .

Main duties of the job
Undertaking medicines reconciliation on admission. Review medication to ensure safe and effective use. Formulation of individual pharmaceutical care plans. Review and update of care plans depending on response, results ofinvestigations and adverse effects. Therapeutic drug monitoring for specific drugs. Educate patients about their drug therapy. Develop evidence based treatment protocols for use either within the directorate or across the Trust where appropriate. Facilitate implementation of local and national guidelines where appropriate.

Job description
Job responsibilities
Provide specialist pharmaceutical information to medical and nursing staffand patients on the designated ward(s) and to General Practitioners andother staff within the Trust where appropriate. Participate in consultant ward rounds and multidisciplinary meetings relevant to the designated ward(s). To participate in the therapeutic drug monitoring service provided inaccordance with departmental procedure. Undertake clinical audit and practice research within the PharmacyDepartment. Record regularly clinical interventions and activities in line with departmental policy. Provide professional and legal clinical supervision and act as the designatedResponsible Pharmacist in the dispensary as allocated. Prepare the weekly pharmacist dispensary cover rota in line withdepartmental policy. Investigate any medicines related incidents or complaints from the designated clinical area and explore ways of minimising risk of future re-occurrence orrisk to staff. Liaise with appropriate health care professionals within Primary Care Truststo ensure the effective and efficient use of resources across theprimary/secondary care interface and where necessary develop shared careprotocols.
