We are looking for an experienced, enthusiastic, and motivated Band 5 Nurse to join our Stroke Community Therapy Team.
Within this role, you will act as one of our Community Stroke Team nurses. You will work closely with our B6 nurse and multidisciplinary team to provide specialist Stroke skills and knowledge to the Community Stroke Team, to deliver an efficient and high standard service to our stroke patients across the New Forest.
The successful candidate will also support with the provision of rehabilitation. The clinical element to this role will involve completion of welcome home, halfway, nursing and discharge assessments for our Community Stroke Team, as well as 6 month and 6 week stroke reviews.
This is a new role, previous stroke/neuro experience is desirable, but not essential. The successful candidate will complete stroke specific competencies and be provided with training. They will also participate in our IST programme, supervision structure, service development and our weekend rota supporting our 7 day a week ESD service. The Community Stroke Team is a 7-day service and our core hours are , but we do consider flexible working between the hours of 8-6 depending on the service need.
The role is Community focussed but based within Lymington New Forest Hospital.
Main duties of the job
To provide effective evidence based care to stroke patients and their carers within National and local guidelines. To be able to carry out key holistic stroke patient assessments, Welcome Home, Halfway and Discharge. To provide stroke specific knowledge and skills within the stroke services and to the wider organisation. To provide education to patients and their carers on secondary prevention, risk factors and medication. Act as patients advocate. Work clinically in the Community. To participate in completing 6 week and 6 month stroke reviews for all stroke patients within area. To share the responsibility of covering a 7 day rota, may include covering from 08:00hrs to 20:00hrs and changing duties at the last minute in order to support staff leave and sickness. Identify training needs as part of continuing professional development. To work within the policies and protocols of Southern Health. To be clinically skilled and up to date in all mandatory training requirements. To assist in the delivery of effective services to patients within the community Stroke Team, utilising evidence- based approach. To support the Community Stroke Team coordinator with referrals and planning. May involve making autonomous decisions regarding accepting patients and organisation of caseload.

Qualifications
Essential
RGN Educated to diploma level
Desirable
Degree level education Stroke/Neuro Specific Training Awareness and understanding of clinical effectiveness including the audit process.
Experience
Essential
Recent experience in a rehab setting inpatient and/or community
Desirable
Relevant post registration experience in Stroke and/or Neuro rehabilitation
Additional Criteria
Essential
Awareness of the specific needs of patients within the community. Competent in computer skills. Daily use of VDU Good communication skills (verbal & non verbal) required across a multidisciplinary team including patients, families and external services Self-motivated Problem solving skills Car driver. Full driving licence required
Desirable
Competent in the use of IT systems, including Rio, EDocs, PAS, CHARTS, Microsoft office. Awareness of the specific needs of Stroke patients-
