AboutThe Role
The Bank Nurse Coordinator plays a vital role within theLawrence Nurses. Acting as the central point of contact for end oflife patients and their families, carers and the Primary Care Team, you will beinvolved throughout their palliative care journey and ensure that that thepatient is supported. Guided by the patients needs and wishes, the Nursingteam are able offer truly holistic and tailored care thats right for theindividual. No two days are the same making this an exciting and challengingrole.

Main duties of the job
Thepost holder will be required to:
Be responsible for new patient referrals andundertaking assessments for those patients that meet Lawrence Nurses Act as a central contact point for the serviceand support the patient, their family and carers throughout their episode ofcare Coordinate, allocate and support Lawrence Nurses, where necessary at the patients home, seeking advicefrom the Registered Nurse Manager as needed To provide clinical leadership and beresponsible for the overall management of patient caseload. To work autonomously within the limits ofexisting knowledge and skills, demonstrating sound judgement and being able tocommunicate in a sensitive manner along with an ability to work effectivelywithin a multidisciplinary team. To offer end of life care and symptom control To offer 24 hour on-call assistance to supportnursing staff To offer support to the patients families andcarers between 8am and 10pm. To be able to respond quickly to urgent On Callcontacts, which may include travel to a patients home. This role allows for 8 working hours to beworked with some flexibility throughout the day.

Founded in 1999 in memory of the late Dr Martin Lawrence, Lawrence Nurses provides excellent holistic care for end of life patients who wish to remain within their home environment. Based in Chipping Norton, and registered with the Care Quality Commission (rating of good), the service is provided in the local area by a team of dedicated nurses who work in partnership with the patient, their family, carers and the healthcare services in the area. We have a partnership agreement with Oxford Health Community Nursing Service.

Key Responsibilities
BeingOn Call at the times agreed and being contactable and available 24/7 to fieldcalls when duty Nurse Coordinator (NC). Respondingto enquiries about our service, where necessary signposting callers to their GPor to other organisations able to help them, including our sister charities. Takingnew referrals, obtaining information about patients likely needs and makingthe decision about whether or not they meet LHNT criteria. If needed,consulting the Registered Nurse Manager (RNM) or Clinical Trustees. Completinga risk assessment before an assessment visit. Undertakingassessments of new patients whose referral meets our criteria, deciding whatnursing support to put in place and deploying individual nurses with as muchnotice as practicable to undertake shifts. Ensuringthat green folder Lawrence Nurses notes are within a patients home as soon as practicableand always before a nurse visits. Liaisingwith the Community Nurses (CNs) and others such as the Community Matrons toensure that all the necessary equipment, medication and documentation are inplace. Carryingout patient risk assessments. Managingthe whole patient caseload, including active, pending and RIP patients. Supportingthe patient, their family and carers by being available to talk when needed andobtaining feedback about how we can meet their ongoing care needs and seekingguidance from RNM where issues arise. Supporting Lawrence Nurses, where necessary at patients home, seeking advice from the RNM asneeded, and giving a thorough handover to the deployed nurse and obtaininghandover information at the end of nurses shifts Keepingthe RNM, other NCs and Clinical Directors briefed about significant issues,seeking guidance as needed and ensuring that this is relayed to all nursesinvolved. Completingall follow up actions after RIP of patient. Handlinginitially any negative feedback or complaints following the procedures set outin the Lawrence Nurses Complaints Policy and of any allegations of abuse as set out inOxfordshire Safeguarding guidelines. Completingdocumentation before end of duty period and briefing of next duty NC. Keepingthe activity tracker up to date and completing any audits deemed appropriate. Beingaware of boundaries and referring to specialist palliative care if they areinvolved in a patients care. Beingthe first port of call for nurses working as lone workers and ensuring theirsafety where possible. Beinga lifelong learner and adhering to the NMC code.This job descriptionis not exhaustive. Qualifications
Essential
RN Registration Level 1 or 2, current NMC registration with relevant post registration qualification, ENB 931or Diploma in Palliative care or equivalent
Desirable
Degree in Palliative /cancer care or equivalent.
Experience
Essential
The minimum of two years recent experience in palliative care either in the NHS or other relevant care setting. Recent experience of working in the community
Knowledge and skills
Essential
Up to date awareness of the skills and knowledge and competencies required for patients with palliative care needs Up to date awareness and understanding of specific needs of palliative care patients in the terminal phase. Communicate with people in a diplomatic and tactful manner. Ability to work alone in a complex and stressful environment and demonstrate appropriate coping mechanisms. Ability to quickly identify and solve problems and take action to address issues Ability to use own initiative and preparedness to seek advice when necessary.
