Job summary

The Dementia and Delirium Team is looking for a highly motivated individual to join our team in supporting the education in all aspects of Dementia and Delirium care within the Somerset NHS Foundation Trust.

You will be responsible for supporting and providing education in classroom style settings and in clinical practice. This will involve training the unregistered workforce, as well as registered health professionals. You will play a key part in supporting the delivery of the Care Certificate programme, new nurse induction, dementia champions as well as specially targeted training within dementia and delirium to various teams and individuals across the trust.

The Education Support Worker will support with the delivery of education to various employees with an aim to deliver or improve high quality, caring and compassionate person-centred care. This will be under the direction and supervision of the Educator/Facilitator and Dementia and Delirium Trainee ACP and Specialist Matron. The role involves working independently and working as part of the multi-disciplinary team.

Some knowledge of dementia and delirium is preferred as well as a passion for caring for those living with dementia and the delivery of person-centred care.

Main duties of the job

Training is the primary function of this role:

To work with and support the educator facilitator to identify core clinical skills for dementia and delirium, to support delivery of tiered training to individuals, groups or teams providing care to patients with these diagnoses.

To deliver clinical skills training to staff working within the acute hospitals and community hospitals.

To support with and review training packages in specific clinical skill areas within dementia and delirium care and support the educator facilitator to develop new training packages appropriate for the setting.

To work with care providers to plan and coordinate training to enable staff to gain a better understanding to enable improved, and effective person-centred care to those living with dementia and/or delirium.

To support with monitoring the training and the service needs and auditing feedback across the trust, to be creative, confident in presenting and using various IT platforms.

The role will include supporting the Educator/ Facilitator to present and assist with training across all sites, Musgrove Park Hospital, Yeovil District Hospital and some community hospitals.
