Job summary

You will be joining a passionate and committed team of Day One Caseworkers based in other Major Trauma Centres, driven to make a difference to the recovery of those impacted by catastrophic injury. Working alongside the Trauma and Rehabilitation Coordinators at the hospital, Day One Caseworkers deliver an on-site, bedside signposting and support service to identify and address any issues and concerns caused by the impact of major trauma. You will take referrals, complete initial assessments of need, support discharge planning and expertly navigate and signpost people in the aftermath of injury, including talking about and facilitating early access to legal support to aid rehabilitation. You will also play an active role in building relationships and promoting the work of Day One within the hospital and wider community, including occasional conferences, fundraising, networking and other awareness-raising events.

Main duties of the job

Work closely, and in partnership, with NHS clinical staff and relevant community and voluntary sector organisations, to address the needs of patients, and those closest to them, affected by catastrophic injury. Operate a case management approach to individuals, assessing and identifying needs, putting support in place including signposting, making referrals and direct support. Provide consistency in assessment of all patients, irrespective of injury cause, age, status, giving access to the earliest possible specialist legal advice which supports rehabilitation and NHS cost recovery. Maintain detailed case records, including accurate records of activity and intervention, using Day Ones Customer Relationship Management (CRM) system, capturing and storing data in line with Day One policies and procedures and regulatory requirements. Implement processes and procedures to collect patient and family feedback and contribute to ongoing monitoring and evaluation of Day One services through providing relevant information, case studies and reports. Deliver awareness raising presentations to clinical colleagues and departments. Organise and facilitate outpatient and in person legal clinic drop-in sessions. Build awareness of the role and charitys purpose through building strong and effective relationships across the regional major trauma network, ensuring those who need it have access to Day One support.

Job responsibilities

Caseworker Northern Care Alliance, Salford & Greater Manchester

Day One Trauma Support

Salary: £33k- £39k per annum, depending on experience and qualifications

Contract: Full time ( hours per week), permanent, subject to a six-month probationary period.

Location: Based in, and primarily focussed on, the Royal Victoria Infirmary, Newcastle but with a requirement to work in other appropriate clinical settings and across the Northern Major Trauma Network as needed.

About you:

You will have resilience and adaptability, understand the importance of confidentiality, safeguarding and professional boundaries; have excellent communication and active listening skills and enjoy the challenge of a busy caseload. You will be an outcome-focussed self-starter, able to work using your own initiative and demonstrate excellent organisational skills with the ability to work in a fast-paced clinical environment. You may have a clinical background, case management experience and/or knowledge of Greater Manchesters community and voluntary sector

The principals of safeguarding, confidentiality and data protection. IT software and systems and programmes, Microsoft office suite and CRM systems. Skills, abilities & qualities: Aligns with the values of Day One. Has a professional, caring, sensitive and responsive approach. Ability to work consistently and safely within policies and procedures whilst maintaining professional boundaries at all times. Emotional intelligence, resilience and adaptability. Desirable

Knowledge of working with or within NHS Trusts and/ or the charity or voluntary sector. Experience of developing and embedding new services or initiatives. Experience of working with people impacted by serious injuries or major trauma in any setting. Previous experience of working with volunteers. A relevant professional qualification is desirable, but not essential. Commitment to, and evidence of, continuing personal development
