Position Overview: We are seeking a skilled and compassionate social Worker to provide specialized care to individuals within the preparing for adulthood team and Transitions The ideal candidate will have a strong background in social work and a commitment to support planning and preparing for adult tasks.
Key Responsibilities:
The Preparing for adulthood team and Transitions are looking for forward thinking social workers to join the adult social care team in order to deliver statutory assessments. support planning and preparing for adult tasks. There are two types of needs that we work to meet within the team and that is to support needs of young people who have a diagnosed Global Learning Disability and require ongoing social care into adulthood. We are also looking for social workers who possess skills to support the needs of young people who may be care leavers and may have a need for ongoing social care due to needs around SEND.
Requirements and qualifications required for this role:
- Current and valid registration with the SWE.
- Proven experience as a social worker.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing high-quality patient care.
