Who You Are
- Possess a professional occupational therapy qualification recognized by HCPC and maintain current HCPC registration.
- Demonstrated experience in completing assessments, reviews, and working within community settings.
- Proficient in preparing needs-led assessments/reassessments and support plans, and utilizing IT systems.
- Skilled in handling safeguarding issues and conducting Best Interest assessments and mental capacity assessments.
- Experience in multi-disciplinary teams in social care or health settings, understanding community equipment and adaptations, including the Disabled Facilities Grant, manual handling, and risk assessment.
- Adept in legal frameworks relating to social care, including the Care Act, Mental Capacity Act, and Mental Health Act, with knowledge of safeguarding and Liberty Protection Safeguards.
- Effective communicator, both orally and in writing, with strong influencing and negotiating skills.
- Committed to equality of opportunity, acting as a role model and upholding high personal and professional standards.
- Adaptable, demonstrating flexibility and a proactive approach in a dynamic work environment.
What the Job Involves
- Deliver effective adult social care services, improving outcomes for people and carers through high-quality, holistic assessments.
- Utilize a strengths-based, person-centered approach, encouraging independence and optimizing individual abilities.
- Manage caseloads, utilizing specialist knowledge for interventions, assessment, care planning, and safeguarding investigations.
- Work collaboratively with health services and partners to ensure timely hospital discharges and prevent admissions.
- Develop and maintain professional standards, promoting integrated working and effective communication within the team and with external partners.
- Conduct complex, person-centered assessments, including Liberty Protection Safeguard assessments, developing rehabilitation plans to promote choice and independence.
- Support personal budget management and resource allocation to maintain independence.
- Ensure compliance with relevant legislation, safeguarding policies, and departmental procedures.
- Provide supervision, support, and mentorship to unqualified staff and students, fostering professional development.
- Utilize management information systems for effective data management, ensuring accuracy and timeliness in records and reports.
- Engage in continuous professional development, maintaining up-to-date knowledge and participating in supervision and training opportunities.
- This is a maternity cover post for 5 months, requiring manual handling experience and risk assessment proficiency.
