  • To link with healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community.
  • To be actively involved in the fortnightly Colorectal MDT meeting.
  • To work in collaboration with the wider MDT
  • Our team provides care and support for patients at all points on the surgical pathway from preassessment consultation/counselling, in-patient and post discharge follow up calls and support to all patients having major surgery.
  • The ability to work autonomously and as part of a wider team is essential in this position, and effective time management skills will be required.
  • Education of patients, carers and ward staff is also a large part of our role, and the successful candidate will be committed to providing teaching and act as a mentor to junior members of staff.
  • The post-holder will support the development and evaluation of the colorectal cancer service using the Key worker role to improve and maintain standards of patient care and promote a seamless service within a multi-disciplinary framework.
