Its what delivering great healthcare should feel like.As a Social Worker you will be:Conducting referrals to LADO and Local Authority in line with safeguarding and assessmentsEducating Hospital staff about relevant safeguarding matters.Completing a full annual social care reassessment within the CPA format.Facilitating access to welfare rights, housing, advocacy services and associated support servicesParticipating in 1:1 supervisionTo be successful in this role, you'll need:A Social Work degreeSocial Work England RegistrationExperience within similar environmentExperience with working with vulnerable adultsWhere you will be working:Location: Thornford Park, Crookham Hill, Thatcham, Berkshire, RG19 8ETYou will work at Thornford Park which is a combination of medium secure and low secure units and rehabilitation flats. The service provides a seamless internal care pathway supporting and empowering patients in their recovery and rehabilitation.You will work alongside the multidisciplinary to provide personalised assessment and treatment programmes for males with mental illness/ complex care needs as well as those with personality disorder.
