Advanced Clinical Practitioner

The ACP has a trust-wide responsibility to provide clinical excellence through the four pillars of advanced clinical practice. The clinical practice pillar accounts for a minimum of 80% of their contracted hours of work and they practice with a significant higher level of responsibility and autonomy which is comparable to that of the level of medical staff providing the same level of service: Practice as an exemplary role model throughout all four pillars of advance practice. Advanced knowledge, skills and judgement for high level and complex decision making in an extended scope of practice. Completing a full holistic health assessment using a range of assessment methods whilst working with patients, relatives and carers.
Request imaging and investigations to underpin and support clinical decisions and diagnosis. Make independent diagnostic decisions based on the above and/or pathology or imaging results. Make complex clinical decisions based on sound evidence base practice and clinical expertise. Initiate prescribed treatments and medication, therapies, lifestyle advice and care.
Screen patients for early signs of disease and risk factors pertinent to their area of practice. Carry out invasive and non-invasive diagnostic and therapeutic procedures where appropriate. Refer to other health professionals as appropriate. Discharge patients from hospital/services as required.
Maintain meticulous evidence of clinical specific competencies of advanced skills. Exercise risk management using professional judgement.
