This is a vital role in ensuring that patients do not fall through the net in long-term follow up To follow up patients who have been discharged from hospital to ensure all discharge actions have been carried out and to identify any additional needs to reduce the risk of readmission Support proactive case finding using population health intelligence data Work closely with professionals from a variety of health specialties to set up patient appointments and treatment plans Attend ongoing training and educational courses to keep abreast of new developments in health care Treat patients with empathy and respect and conduct oneself in a professional manner at all times Comply with organisation guidelines and health care guidance and regulation
