We require an experienced or newly qualified Band 5 RMN who can bring their own professional expertise to our fast-paced team, and who are confident in managing risk in the least restrictive way with support from the team. By its nature, crisis work requires professionals who have a broad range of skills including being skilled mental health assessors with an ability to signpost service users towards the most effective treatment pathways.

The Crisis Response and Home Treatment Team in Buckinghamshire Mental Health Services is a key part of the Urgent Care Pathway meeting the demands of the Ten-Year Plan to provide Crisis Response and Home Treatment Services. Clinicians employed into the team are part of an exciting and developing service aimed at supporting our most vulnerable patients and their families in the least restrictive setting possible, by providing a variety of highly intensive interventions at home.

Service Users access a crisis response in a variety of ways, ranging from referrals from their GP and attendance at an emergency department, to when they need care in the community and are seen by the police and our street triage teams. The aim of the Urgent Care Pathway is to bring together all those access points under one team, including:
Psychiatric Liaison
Overnight Response Team
Assessment Teams
Street Triage and Health Based Places of Safety

The post holder will be mainly based with the online ATP within the CRHTT to provide assessment, signposting and short-term treatment interventions where required, to all referred service users. ATP is an online DBT-based group programme, each group is facilitated by 2 staff and covers a wide range of topics / interventions. Services users who present in crisis and would benefit from DBT based group work are supported for 2 weeks through the ATP programme.

The post holder will work within the Recovery Model and will play a key role in developing the interventions required for the CORE Crisis Fidelity model. They will work as part of our team managing the care for a caseload of patients, liaising with other professionals and reviewing the patients care under Care Programme Approach. Additionally, they will provide support to service users in crisis when required. This will become the core business of the team. The practitioner will also work with the team in delivering our responsibilities under the Care Act 2014 with regards the assessment and care planning of Social Care needs.

The successful applicants will participate in a duty rota of early, late or long day shifts covering 7 days a week and we are committed to supporting staff to balance home and work life with flexible working options. The role is predominantly based in High Wycombe and will require some wider travel within the county.

1. To participate in facilitating the delivery of the Online Acute Treatment Programme as a crisis intervention.
2. To triage new referrals into the team with support from senior colleagues and our consultant psychiatrist and to manage them with an appropriate level of urgency in relation to the presenting risk.
3. To work under the direction and supervision of a senior practitioner as part of a multi-disciplinary team to, assess, plan and implement patient specific programmes of care/interventions under the Care Programme Approach, contributing to the continuous assessment of patients.
1. To participate in all the decision making processes but not be the lone decision maker.
1. To carry out the initial assessments, with support and supervision from senior colleagues of service users who meet the referral criteria of the Urgent care pathway / CMHT and to undertake short term treatment where appropriate.
2. To manage, formulate and implement treatment plans, delivering care under the clinical model.
3. To evaluate effectiveness of care / treatment plans whilst working within the framework of CPA and Risk Assessment.
1. To have a working knowledge and understanding of the relevant legal frameworks: Mental Health Act, Mental Capacity Act, Care Act 2014, and fulfil the statutory duties of the NHS and the local authority.
1. To report all relevant information to the referring agent and keep them informed of the service users progress.
2. To assess, plan, set goals with the service user, carers and families; implement and evaluate care using psycho-social interventions
