Assessment and Enablement Worker - Community Wellingborough Hub
18.5 hours per week (pro-rata), Working Pattern: Monday, Tuesday and Wednesday AM
The North Northants Council Community Hubs are on an ambitious and exciting journey to ensure that customers locally achieve the best outcomes at the right time, and with the right support.
As part of our transformation we are focussed on assessments and support planning that identify an individual’s strengths and the outcomes they want to achieve.
The Assessment and Enablement role in the team will be to undertake detailed assessments to ensure that there is no delay in enabling people to move accommodation, make changes to their lives or ensuring providers are meeting identified outcomes set by, and for the customer. The assessments are in line with statutory requirements and best practice.
We are looking for a highly motivated, organised and creative person with experience of working within a social care background. You will have a commitment to strengths based, person-centred values and have good communication and IT skills.
