We are looking to recruit to our Carers Assessment Team. This is a hybrid role with working from home options alongside our Wellingborough base.

Carer Support Navigators offer Carers Assessments and support to Carers in Northamptonshire in order to enable the sustainment of the caring role, reduce the impact of caring on Carer’s health and wellbeing and to prevent crisis. By ‘Carers’ we are referring to informal carers who look after a friend or family member with a disability or long-term illness out of a sense of duty and compassion as opposed to a care worker who is employed in their role. This role will focus on being a part of our Adult Assessment Team where you will be supporting Carers primarily in the community, and by telephone as part of our Carer Support Line Team to provide the best possible support and outcomes for Carers.

**Job Dimensions**
- To provide an informed and professional response to carers through a confidential service of Carer-centred assessment, support and crisis planning.
- To provide advice and information alongside emotional support.
- To work one day a week on the Carer Support Line.
- To provide short term intervention plans for Carers where appropriate ensuring they are fully supported by effectively managing risk and responsibilities, using a person centred approach.
- To maintain accurate written and concise computer records in accordance with local policies and data protection and confidentiality legislation.
- To manage own caseload and have the ability to prioritise tasks and responsibilities whilst ensuring assessments are completed within an agreed timeframe.
- To work in partnership to improve collaboration and coordination to achieve the best possible outcomes for Carers.
- To work as part of a team, participating in team meetings, team building, training and activities.
- To prepare for and participate in regular supervision meetings and to seek additional supervision as required.
- To demonstrate awareness/understanding of equal opportunities and other people’s behavioural, physical, social and welfare needs.
- To ensure that reasonable care is taken at all times for the health, safety and welfare of yourself and other persons, and to comply with the policies and procedures relating to health and safety and safeguarding within the organisation.
- To undertake developmental tasks and carer awareness-raising as required. This may include being involved with fund-raising however this will be discussed with the worker on a case-by-case basis.
- To carry out any other duties which fall within the broad spirit, scope and purpose of this job description and which are commensurate with the grade of the post.
