**JOB TITLE: Support North Northants: SNN Local Coordinator (x2)**

**HOURS: 37 hours per week**

**Contract Length: to end of March 2025 (with a view to continuation)**

**LOCATION BASE: 1 Onsite and community based in East Northants and Wellingborough and 1 within the whole North Northants area.**

**SALARY: £29,133 per annum**

**RESPONSIBLE TO: SNN Team Leader**

**BACKGROUND**

**We are putting transformation in practice and embarking on an exciting new way of working between partners. Led by the North Northants voluntary, community, and social enterprise (VCSE) sector we are working with the Integrated Care Service (ICS) to transform how services are delivered.**

**People and communities are facing real challenges due to the cost-of-living crisis post Covid and social care and health services are under significant pressure. We want people in local communities to have easy access to support, have support early and we want to prevent people’s needs getting worse.**

**The service motto is: ‘We don’t give up on people’.**

**Support North Northants (SNN) is a collaborative service model, led by the voluntary sector, that can support people for a post-Covid world where people can have access to integrated and preventative services. This will help to reduce pressures on statutory services and develop higher levels of community resilience.**

**The model helps to manage demand and pressures in health services, and develops a placed based, integrated care, model that ‘catches people early.’ It enables the VCSE to provide easier navigation to support pathways and wrap around services that meets both existing and new, or emerging, needs.**

**The strength of the model is that it will continue to be, co-designed and co-produced with people and we will work in collaboration with all ICS partners with the aim of achieving the best outcomes for people quickly and effectively.**

**The outcomes for the service will be developed with people based on the ICS’s ‘Live you Best Life’ 10 ambitions**:

**SUMMARY OF POSITION**

**We are looking for one SNN Local Coordinator who can change the culture of collaborative working with colleagues employed across the voluntary, statutory and private sectors, and ensure people can ‘tell their story once’.**

**The SNN Local Co-ordinators will support the Single Point of Access (SPA) to play a crucial part in facilitating person-centred, holistic support for people when they first present for support and ensuring that people ‘get to the right place first time’. The Single Point of Access will work closely with other services delivered by the VCSE and statutory services to achieve the best outcomes for people.**

**The SNN Local Co-ordinators will be case managers and line manage the Support Workers for their localities.**

** MAIN DUTIES**:
**1.** **Raise the profile and awareness of Support North Northants, establishing strong links with colleagues across the sector.**

**2.** **Lead and promote the Single Point of Access and provide an accessible service and support for people and communities.**

**3.** **Provide appropriate information and guidance promoting the culture of independence and positive outcomes.**

**4.** **Obtain an accurate, person-centred picture of the outcomes people need, working with the family, carers and other agencies to ensure they tell their story once.**

**5.** **Assess risk on an individual basis and facilitate services where appropriate.**

**6.** **Identify key partners who can support the person and develop joint support plans.**

**7.** **Present people to the CATCH team and ensure relevant partners act to develop a support plan.**

**8.** **Work with SNN aligned services to ensure access to wide range of support.**
