**Service Support Worker - Assisted Discharge - Craigavon Area Hospital**
**Contract type: Fixed Term**
**Hours per week: 21 hrs, 12 noon - 20:00pm, Tuesday to Thursday**
**Salary: £17,464 Per Annum ( Actual Salary is £10,478 for 21 hrs )**

We are looking for a passionate Service Support Worker to join our Craigavon Area Hospital Assisted Discharge Service. The Independent Living Service Support Worker post will provide reactive, focused and flexible support to inpatients following a hospitalstay to enable early discharge when clinical needs have been met. The post will assist the Independent Living Service Coordinator and the Hospital Pathway Team by undertaking duties required to meet the service users' needs in order to re-enable service usersto remain in their discharge location and prevent hospital re-admission. Referrals may require the support worker to take the patient home from hospital to enable their discharge.
The post-holder will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual. The post-holder will be providing practical and emotional support in the service user's discharge location and signpostingto other services where appropriate to enable the individual to live independently in their accommodation.
The Support Worker will act as contingency for the Service Coordinator when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area. Service confidentiality and safeguarding vulnerable people are keyresponsibilities of the role. The post requires weekend shifts and some evening shifts ( Majority of the shifts will be 12 Noon -20:00pm).
This post does not include personal care and management of medication.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR)supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships acrosshealth, social care, and emergency services.
