Service Coordinator
The Service Lead and Developer will be fully responsible for the operational management of a staff team and a number of clients receiving care and support within the community. You will work closely with social work teams to promote Aspire as a high-quality provider of services and promote excellent relationships with all referral sources, following up referrals quickly and making every effort to grow the service areas they are responsible for.
You will assess individuals’ needs, their aspirations and desired outcomes and use this information in the planning, co-ordinating and management of each individual’s service, ensuring consistent delivery of the appropriate high-quality support and care.
You will work closely with their staff teams and develop, nurture, and manage them effectively. You will demonstrate and promote excellent oral and written communication to ensure that the most up to date information is known by relevant parties in relation to the provision of care.
This is a fast-paced role and you should be able to respond flexibly to service demands, ensuring that individuals have maximum opportunities in directing their care to live as fully, inclusively and independently as possible as valued citizens in the community.
You will create rota's for your services and teams and it is important that you maintain meticulous record keeping for both their team and clients and so a high level of personal organisation and administration skills are important.
Although this role is office based, there is a regular requirement to visit the homes of clients and prospective clients, social work offices and any other business location of Aspire. As such, you need to hold a current UK manual Driver’s Licence with access to their own car. Participation in the on-call activity is expected, for which there will be an additional monthly payment of £145.
