Ordering and sourcing of up to date and relevant patient information, liaising with the Macmillan information centre where appropriate. Making and assisting with referrals to other agencies and services. Support the Breast CNS team in the collection of data to support audits and patient surveys, and to keep patient records up to date. To be able to complete a HNA and where appropriate a care plan, managing immediate concerns under support of the CNS team where needed.
Under the supervision of the CNS team process and feedback HNA data to site specific clinical teams and Cancer Care Review. To develop skills and an understanding of when patients have complex needs which need to be dealt with by the CNS, and those which can be dealt with by the Assistant Practitioner. An awareness and basic understanding of treatments and their side effects such as chemotherapy, radiotherapy, immunotherapy and hormone treatments. Under supervision of the Breast CNS team, to prep notes, and review MDT outcomes to forward think and plan for future clinics and future patient contact An awareness and understanding of the remote monitoring service, to assist in maintaining the service when other members of staff are on leave/absent.
Organising and coordinating the support group/Breast Webpage and information. Planning and organising health and wellbeing events, working with the cancer support centre in doing so where appropriate. Clinical skills including, breast wound management / aspiration of seromas following appropriate training. Mapping local support and engaging with health promotion activities.
Provide education and support to junior staff and other clinical areas. Engage in opportunities to scope current ways of working to be able to implement future service development. High level of communication skills to communicate with patients, friends and relatives and involve them in care and decision making as appropriate. To offer psychological support to patients Working Pattern: 22.5 hours per week, over 3 days.
