Due to maternity leave, we have an exciting development opportunity for a dynamic and experienced Band 7 Occupational Therapist, who is looking to further develop specialist clinical and leadership skills, or someone who is looking for the right experience to take the next career step. This is an 8A Clinical Specialist OT role; the post is also that of Service Lead within Brent community rehabilitation services, including general and neurological rehab, and Falls.

The post holder must have expert knowledge in the fields of rehabilitation, especially neurology, and should have experience of leadership at Band 7 level. Exposure to waiting list management, service delivery issues/ considerations, and finance, would be advantageous. You will have direct line management responsibilities for OT’s, Rehabilitation Assistants, and Clinical coordinators within the rehab service, as well as Falls Team Lead Physiotherapist.

You will be responsible for monitoring the performance of the services and the team and will work closely with the Clinical Business Unit Manager and Clinical Specialist Physiotherapist, to ensure safe and effective service delivery.
- Provide, maintain and review a comprehensive client-focused, highly specialist Occupational Therapy service for adults using Brent CLCH Community Rehabilitation Services. This service will involve clients, carers, families and service providers.
- Provide clinical leadership in the area of neurological rehabilitation, general rehabilitation and falls, covering a range of disorders.To act in a consultancy role for clinical staff in addressing complex service user issues in rehabilitation
- To hold the strategic overview and lead on the development of community rehabilitation services pathway provided by Brent CLCH Rehabilitation Services. To work in collaboration with external partners throughout the patient pathway ensuring the best outcomes for service users
- To be responsible for ensuring evidenced based practise through adherence to current clinical guidelinesto ensure the most appropriate and cost effective clinical pathway for individual clients.

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.
1. To ensure achievement of key performance indicators for Community Rehabilitation Services and Falls
2. Undertake uni-professional, multi-professional and specialist Occupational Therapy assessments using evidence-based outcome measures, in a home or clinic setting, of clients referred to Brent CLCH Rehabilitation Services. Neuro clients may have complex conditions including physical and cognitive impairments as a result of an acquired brain injury (ABI) including traumatic brain injury and Stroke or Long term neurological conditions including Multiple Sclerosis, Motor Neurone Disease, Parkinsons Disease and Spinal Cord Injury
3. Following specialist assessment, to use advanced clinical reasoning, analysis, assessment and re-assessment skills and using a client-centred goal setting approach, to provide individual specialist expert Occupational Therapy treatments and programmes. This will include physical, cognitive, perceptual, psychological and social integration, vocational and leisure needs and well being of the individual. The post-holder must have an extensive knowledge of cognitive, physical and psychological impairments, in order to assess and decide on the best form of implementation of treatment and communication with clients
4. Carry out patient education, home programmes and intervention using a range of treatment modalities, taking into account the latest evidence based practice and research published as indicated in the Royal College of Occupational Therapists (RCOT) and local standards and protocols
5. Work with consultants and practitioners in specialist rehabilitation units offering expert opinion to identify, agree and implement rehabilitation goals following discharge from hospital
6. Through advanced clinical reasoning and analysis, to identify clients needs for input from outside agencies and to liaise and refer clients to these services including other healthcare services and healthcare professionals and voluntary agencies, as appropriate
7. Recommend specialist equipment to meet complex client’s needs and educate clients in the safe use of this equipment in order to maximise client’s independence. This will include acting in an expert consultancy role with MDT members to advise in complex cases.
