**About the role**
You will work in the community as part of a multidisciplinary team offering proactive, holistic care and support working within Wrekin Primary Care Network to help deliver the new Shropshire Telford and Wrekin Dementia Strategy.
The Telford and Shropshire Dementia Support Service provides a highly responsive and individualised information, signposting, and referral service to people affected by dementia. You will:
- Provide personalised support to people with dementia, their families and Carers to take control of their health and wellbeing, live independently, improve their health outcomes and maintain a healthy lifestyle
- Assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improving their sense of well-being, and putting them in more control of their lives
- Co-produce a simple personalised support plan to address the person with dementia’s health and wellbeing needs by introducing or reconnecting people to community groups and statutory services
- Evaluate how far the actions in the care and support plan are meeting the person with dementia’s health and wellbeing needs
- Communicate with people with dementia and their carer’s, asking questions and actively listening to problems to really understand, so they can answer questions and give families the support they need to tackle problems
- Develop trusting relationships by giving people time and focus on ‘what matters to them’
- Take a holistic approach, based on the person with dementia’s priorities and the wider determinants of health
- Work collaboratively with other health and social care colleagues, being knowledgeable about local third sector services and other local community support available for people with dementia and their carers
- Support the development of Dementia Friendly GP practices and community resources
- Support couples to understand dementia and its impact on relationships (through CRISP courses)
- Facilitate peer support in market towns/ localities across Shropshire, Telford and Wrekin working collaboratively with other voluntary organisations and health and social care professionals. Enabling people living with dementia, carers and families to decide what they want to do, where they want to meet and how they want to run their groups
- Develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering individuals affected by dementia to make informed choices.
- Guide and navigate people with dementia, carers and families to sources of help and support locally and nationally.
- As members of the PCN’s team of health professionals, take referrals from the PCN’s Core Network Practices and from a wide range of agencies to support the health and wellbeing of people with dementia and their carers
- Be part of a highly motivated and supportive team, sharing responsibility of some admin tasks, including duty telephone coverage.
