End of Life and Bereavement Practitioner

The post holder will have clinical responsibilities within the bereavement service and provide specialist knowledge and care.
They will work with all Trust staff to empower and do all they can to achieve what we would want for our own families, for everyone, and to develop a culture that end of life care and bereavement is everyone’s business.
The service model of bereavement support will be provided in line with NICE Guidance:
Component 1 - Grief is normal after bereavement and most people manage without professional intervention
Component 2 - Some people may require a more formal opportunity to review and reflect on their loss experience, but this does not necessarily have to involve professionals
Component 3 - A minority of people will require specialist interventions
The post holder will provide immediate and on-going emotional support for patients in the last days of life and their families.
They will support families who are bereaved due to expected and unexpected/sudden deaths.
They will ensure onward referral for people with more complex grief.
Support all areas to implement the end of life and bereavement model of care for every person every time.
Ensure that the end of life tools are used as an aid/guidance by staff by ensuring they have a good knowledge and understanding of them.
Provide high quality end of life care to patients and their loved ones as required, either through service need or “role modelling” of good practice
Act as a role model, be respectful and have an ability to challenge staff/senior staff where there is a risk that decision making, for patients at the end of their lives, is being compromised, is unsafe or not in a patient’s best interests.

Contribute to the achievement of ELHT’s objectives and targets for end of life care.
To participate in local and national initiatives to raise awareness of end of life care (e.g. Dying Matters Week).
Work flexibly to provide a 7 day service.
They will assist in the development of evidence based guidelines for use in clinical practice
Work alongside the End of Life and Bereavement Clinical Lead to contribute to the targets and development of local, regional and national end of life care and bereavement initiatives, directives and strategies
Work collaboratively with other bereavement agencies such as the local charities and service providers to ensure families receive appropriate, timely support.
Participate in local and national audit of end of life and palliative care
