**Stavian Ltd** **Job Description and Person Specification - Rehabilitation Support Worker** **Post Background - **Stavian is a Private Brain Injury Case Management provider working across the UK and we have positions available in Bristol as a Rehabilitation Support Worker for a client who was involved in a road traffic accident. Whilst making a full physical recovery, they were left with a significant brain injury. Our client lives in their own home with their spouse and child. They do not require any assistance with personal care, but do need daily support with some domestic tasks, social and vocational activities and implementing a rehabilitation plan. There is a full multidisciplinary team involved in developing ongoing rehabilitation strategies. There are currently two Support Workers in post, but one is leaving and we are looking to replace them and also add to the team. There is a fulltime and a part-time posts available up to 40 hours per week and they involve both weekday and weekend working. The basic rate of pay is between £18 and £20 per hour dependent on experience. We are also looking for someone to take on a supervisory role within this team. Whilst we have ongoing responsibility to coordinate the support and treatment package, successful applicants will be directly employed by our client’s deputyship. This is a solicitor appointed to manage all financial affairs. All contracts are substantive and include a workplace pension scheme. **Salary Scale**: £18.00 to £20 per hour; Sleep-in duties £65.00 per night (only required periodically) **Role Purpose - **To be part of a team providing support which aims to maximise people’s capacity to live independently in the community. Our philosophy is that successful rehabilitation takes place in a person's everyday environment and is relevant to their everyday life. The challenge is to actively support people to do the "simple things in life" when those simple things have become difficult. Rehabilitation is about reaching and maintaining optimum physical, intellectual, psychological and social functioning. It is about providing the right 'tools' and support so that each person can maximise their independence and self - determination. **Role Context - ** _C_ommunity rehabilitation assists people to live as independently as possible in their own home and enables them to live as interesting and self-determined a lifestyle as possible. Community rehabilitation provides people with the tools that will help them to optimise their ability and to live their life the way they want to as far as possible, despite their impairments. This requires helping people set goals, assess their own strengths and weaknesses, plan and direct activity, initiate or avoid certain behavior, monitor current activity and evaluate personal progress. **Key Accountabilities - **are the major ongoing responsibilities of this post; they describe what the person in that position is responsible for regularly producing or providing — i.e. the expected “deliverables” of the position. **Focus of Support**: - To deliver support that is enabling, and which maximises the person’s independence. - To deliver support that is person centred. To view the person as being capable of autonomy and growth. - Providing support that meets the best interests of an individual. - To introduce and sustain the person in their local community and wider society. - To assist in delivering support that is based on best evidence. - To work in an multidisciplinary way with colleagues and contribute to a unified programme of support. - To assist in the development, delivery and evaluation of multidisciplinary support and coaching plans. - **Personal Rehabilitation**: - Effective rehabilitation draws out a person’s potential. The post’s accountabilities come under four main areas:1. Independent Living Skills Development - To deliver independent living skills programmes that help people learn what they need to know to live independently. The key areas may include food shopping, cooking, housekeeping, laundry, budgeting, tenancy issues, accessing local services and supports, stress management, personal care and safety, relationship building, and physical / mental health. - Skill building activities are based on needs as identified through a comprehensive life-skills assessment. - Rehabilitation support can involve a combination of modelling, hands-on experience and one-to-one planned activities. 2. Education, Training and Employment Assistance - To provide support for finding and maintaining vocational engagement that the person finds purposeful and which structures lifestyle and provides valued roles. It can also involve identifying career goals and actively engaging people in career planning. Support must be provided in ways that promote stability, continuity and encouragement as this has been shown to support successful education, training and employment outcomes. 3.
