Under the direction of the Adferiad Registered Manager, the Senior Housing Worker is to work as part
of the team and lone work to support and improve the lives of people with serious mental illness and
co-occurring substance use disorder.

The position of Senior involves taking responsibility of delegated tasks such as the completion of
processing referrals from point of receipt to the delivery of support through the creation of person
- centered/housing related support delivery plans, allocating tasks to staff members. The role will
include acting as Deputy in the Registered Managers absence, completing reviews of support,

recording evidence of outcomes for monitoring, ensuring all client files are up to date and ready for
any validation, and ensuring compliance with internal and external quality standards inspection and
regulation (including Care Inspectorate Wales - CIW). As a deputy, a Senior is expected to be able to
take on any essential admin and oversee the management of Housing Support Workers as directed by
the Registered Manager.

The role will involve working closely with the Registered Manager and team to develop this new
service and to be part of the agreed-On Call Rota.

**About this job:
The provision of 24 hour Supported Accommodation to five occupants who are diagnosed as
experiencing a serious mental illness and co-occurring substance use disorder and have been
assessed as needing high levels of support to help with their recovery.

The service will be staffed 7 days a week, 24 hours per day with on-site waking nights, sleep-in cover.***
The aim of the staff team and this community based supported living service will be to encourage and
enable clients to reach a greater level of independence over a reasonable period, and then to
facilitate their move on to more independent living in the community with the opportunity of
receiving further support from our outreach services.

The project supports a preventative and rabblement agenda and the wellbeing initiatives as identified
within Welsh Government policies and the Social Services and Well-being Act (Wales) 2014.

**Duties and Responsibilities**:
Supporting and assisting occupants with the following**:
Feeling secure within their home.

Increasing independent living skills.

Using appliances safely and maintaining reasonable domestic standards.

Personal finance, budgeting, and accessing benefits.

Mental and physical healthcare.

Self-management of medication.

Attending appointments.

Visitors and their access to individual’s property.

Building and maintaining relationships with neighbours, families, friends, professionals.

Developing skills and interests.

Supporting the Registered Manager in processing of referrals - following procedures for
completing new assessments, risk assessments and creating individual person-centered service
delivery plans.

Follow procedures for conducting 3 monthly and annual reviews - or as and when required due to
changes in individuals’ needs or circumstances, ensuring risk assessments and support delivery
plans are kept up to date and are relevant.

Support the Registered Manager, in monitoring and evaluating staff performance during
supervisions and with ongoing support.

Attendance at multiagency meetings to ensure the complex needs of the client(s) are addressed
appropriately.

Communicate effectively and professionally with individuals receiving service, Support Workers,

Ensure external and internal communication is clear, timely and of high quality.
Data collection and appropriate recording and presentation for monitoring and Quality Assurance

reports as directed by the Registered Manager.
Follow procedures for general record keeping and information collection, saving and archiving
- adhering to confidentiality and data protection policies.
Actively seeking opportunities to promote the services offered by the organisation within the local

community. Including attending events for this purpose and the distribution of locally produced
publicity material.
