**About Myself**

I am looking for a 2 Personal Assistants 2:1 to take me out socialising near where I live, in the local Merthyr area.

I am a 15-year-old teenager I have been diagnosed with Global Development Delay and have a rare heart condition called Wolff Parkinson-White Syndrome (my mother will explain more about this). I struggle with my anxiety and repetitiveness.

I spend most of my time at home with my mother and would like a Personal Assistant to take me out for my Mam to have a break and for me to gain confidence in social settings. I like most activities but have to be careful with my heart condition, so its important that anyone who works with me is **CPR trained.**

Likes:
- I love Art and anything creative
- I love listening to music
- And also likes dancing and playing musical instruments
- I like to going out for food.
- I am chatty and like lots of conversation.

Dislikes:
- I don’t have any dislikes.

**Working Hours/Pay of Rate**

**2 VACANCIES 2:1 SUPPORT**

6 hours per week term time (39 weeks) - £13.00 per hour

8 hours a week school holidays (13 weeks) - £13.00 per hour

Hours need to be flexible to my needs and will be discussed in the interview

**Tasks and Duties**
- I require my personal assistant to take me out of my home environment and get involved with my activities and interests.
- Supporting me on a 2:1 basis
- Support and encourage my independence where possible, and preserve my personal sense of well-being
- Support and encourage me out within my local community
- Any other reasonable tasks required
