As a Recovery Coordinator with Change Grow Live, you’ll work with people who use our services, helping them get to where they want to be.

An exciting opportunity has arisen within the CHART Kirklees service for the role of a Family Recovery Coordinator, whereby you will work with a caseload of service users who have children or young people living in their household. You will be based predominantly in Dewsbury but will need to be flexible to work across Kirklees including in community venues.

**Responsibilities**:
The post holder will have care co-ordination responsibilities alongside the tasks of recovery planning, risk assessing, risk management planning and client-centred reviews. In addition, knowledge and experience of psychosocial interventions, to engage collaboratively alongside colleagues - including peer mentors, volunteers, nurses and doctors, to ensure an integrated package of care.

The post holder will be committed to a positive, holistic and empowering approach to care co-ordination and will work with service users to initiate recovery from drug dependence, to enable service users to build their own recovery capital and to support them to move into long-term sustained recovery and social (re)integration.
