As a Recovery Coordinator with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.

**Responsibilities**:
With this role you’ll be working as part of the Intensive Opiates team and carry a caseload of individuals with Mental Health needs.

Key elements of the role:

- Managing a caseload to provide intensive support to individuals with substance misuse and mental health needs
- To regularly liaise with mental health professionals
- To ensure a MDT approach to caseload management to encourage lead agencies who are involved to actively supporting service users
- Promote harm reduction and personalised health promotion interventions to encourage people to access mainstream services within the community
- Utilise home visits and community venues for one to one and group work sessions
- To provide rapid and flexible interventions where required to meet the needs of the service users
