At Connection Support, our team staff members and volunteers work alongside people to overcome life’s challenges, with the goals of solving homelessness and achieving independence.
We are currently recruiting a Senior Case Worker to join the Out of Hospital Team in North Oxfordshire.
**About the role.**
Working within the community and some attendance within the hospital, you will support people who are experiencing mental health challenges to address their housing issues to avoid homelessness and readmission to hospital.
Providing assertive outreach support, you will support people to increase their confidence, develop routines, build skills and plan for their future. This may include accessing support for accommodation, debt, physical and mental health treatments, substance misuse, engaging in activities, building networks, and maintaining contact with friends and family.
This post is fixed term for 6 months, with the potential for longer, covering maternity leave. Working hours are full time, 37.5 hours per week, Monday - Friday.
Salary £26,649 - £30,396 per annum, dependent on experience.
The closing date is Thursday 10th October. Interviews dates will take place on the 15th and 16th October.
**About you**
You have experience of working with people who have experienced mental health issues, or mental health crisis, and have knowledge of mental health conditions and the impact these can have on daily lives.
Your experience should also include supporting people to resettle into their homes, including managing utilities and rent accounts, ensuring people have furniture, accessing benefits, linking in with community networks and statutory services.
Due to the nature of the role, you will need a current driver’s licence and access to a vehicle.
