You will be joining an MDT consisting of a range of healthcare, social care and third sector staff.
We are looking for dedicated and compassionate registered practitioners (RNMH, Social Worker, OT) who can demonstrate our leadership behaviours of working together, being authentic, taking responsibility and embracing change.
The role is an integral part of the MDT where the practitioner can make a significant positive contribution to a service users recovery journey. As a keyworker, the post holder will assess, plan, implement and evaluate evidence-based care to service users and their carers within a defined geographical area based around each of our primary care networks. The postholder will provide professional advice and support to other agencies and members of the MDT. The postholder will provide supervision for junior members of staff and deputise for the Operations Manager when required.
As part of our transformation we are moving away from individual practitioners holding caseloads to a key-working model where there is rich MDT involvement and shared decision making leading to better service user outcomes.
We can offer a multitude of training opportunities which include Non-Medical Prescribing and training to deliver therapeutic interventions.
CMHTs aim to provide planned and effective mental health support and interventions alongside the service user, their loved ones in line with the goals and needs of the service user. CMHTs work with other agencies and organisations to look after the health of its local population. Responsibility for this is shared between the CMHT, other organisations, the service user and their loved ones. Team members work in collaborative multi-disciplinary teams where they help one another. CMHTs aim to be recovery focussed and work in accordance with trauma informed care principles. The teams include; Consultant Psychiatrists, Clinical Psychologists, Registered Mental Health Practitioners such as, Mental Health Nurses, Occupational Therapists, Community Support workers, Peer Support Workers as well as having representation from social care, housing, drug and alcohol services and the voluntary, community and social enterprise (VCSE) sector. The MDT holds overall responsibility for the care provided with each service user having an identified keyworker in the team who is the point of contact for them and their carer.
NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools.
- The post holder is responsible for the assessment of service user care needs which may be highly complex and the development, implementation and evaluation of programmes of care, without supervision.
- Responsible for ensuring programmes of care are co-produced with service users and carers, are kept up to date and relevant and are available for others within the multidisciplinary team to refer to when needed
- To contribute to the delivery and development of the CMHT by using effective evidence-based practice in line with agreed national standards for clinical practice
- To undertake the role of keyworker for service users whose needs may be highly complex, liaising and sharing information which maybe highly complex and sensitive with others involved in their care and in line with the Consent to Share Information Policy
- To provide structured therapeutic interventions with potentially complex clients in one-to-one session or group settings as appropriate
- To be accountable for the planning and delivery of all aspects of patient care in relation to the service as set out in the NMC Code of Practice.
- To continuously improve clinical care standards in line with the clinical governance framework to ensure the highest quality of care is provided in safest manner
- To work as an autonomous member of a multi-disciplinary team in the provision of specialist care and knowledge
- To communicate effectively with all members of the multi-disciplinary team, patients, carers and the public
