RNs work in an outcome focused environment and work in partnership with PoP, NPS Responsible Officers, and community services to support. One to One Support will be core to service delivery, building trust and undertaking activities and interventions thatare best delivered face to face. The role may be delivered from a custodial and/or community setting.

**Duties;**
- Proactively manage a caseload of Service Users in line with the information contained within the Referral provided by the Responsible Officer.
- Support and move Service Users in addressing their assessed needs in line with the Service delivery model.
- Conduct an assessment with all referred Service Users to determine the appropriate level of support required to address their needs.
- Prepare and agree individual action plans with the Service User.
- Provide intensive support to all Service Users on a one-to-one basis and in group settings dependent on the needs identified by the assessment.
- Bespoke casework to meet individual needs.
- Deliver Intervention Modules (1-2-1 and Group)
- Work with colleagues in partner agencies to achieve successful outcomes
- Work with Support Providers to maximise opportunities for the service user group.
- Contribute to case conferences such as MAPPA where appropriate.
- Refer Service Users to appropriate agencies as required and liaise with those specialists in order to monitor progress made.
- Work with Service Users of varying complexity of need and across each Complexity Level (as applicable)
- Be Alert and responsive to behaviour, information or changes which could indicate a change in Risk of Serious Harm
- Respond appropriately to challenging behaviour (including Unacceptable Behaviour) in order to de-escalate tension, enable Service User to manage strong feelings and to ensure their own safety and that of others.
- Understand risk assessment as compatible with role and grading and to ensure appropriate consideration of risk when placing service users into employment or mainstream provision.
- Achieve outcomes in accordance with contractual KPIs and the Performance Management Framework.
- Contribute to the continuous improvement of Dependancy & Recovery
