District Nursing Sister

We require experienced community specialist practitioner to lead a District Nursing Team based in the Blackburn with Darwen locality of Lancashire & South Cumbria NHS Foundation trust, this will be Part-Time (30 hours per week). The successful applicant will be responsible for providing leadership to an established district nursing team to provide highly skilled nursing care to patients within their own homes.
This is an opportunity for a District Nursing Sister to lead and manage a skill mixed district nursing team as they develop the service to embrace new ways of working.
The post holder will be expected to work flexible hours, covering 08:00am - 20:00pm, 7 days per week as part of a rostered shift system
To provide professional leadership to a District Nursing Team and to facilitate a culture of learning and reflective practice within that team.
To assume continuous responsibility for appropriate deployment and supervision of staff for the effective management of a defined caseload.
To provide clinical leadership in the assessment, planning, implementation and evaluation of high quality evidence based nursing care to patients in their own homes and other community settings, to ensure that the care needs of the caseload are met.
To provide direct clinical care to patients with identified specialist or complex needs.
To work in collaboration with the District Nurse Team Manager to support the relevant national and local initiative and INT / ILT.
To work to a defined shift pattern with a service responsible for the provision of care 24/7 365 days per year.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
