Bolton, United Kingdom Greater Manchester Mental Health NHS Foundation Trust Full time

We have an exciting opportunity for an enthusiastic and committed Carer Support Worker to join our dynamic, professional and friendly team. This role is perfectly suited to someone with experience of working with carers of people experiencing difficulties with their mental health.

Based in the Early Intervention in Psychosis Team/Community Mental Health Team you will work closely with other members of the Multi-Disciplinary Team, in leading the assessment and provision of support to carers, family members and significant others to enable their mental health and resilience to be optimised.

We are seeking a dynamic, innovative, enthusiastic individual who is committed to working with carers within the community setting.

As a Carer Support Worker you will identify carers coming into the service and will ensure they have been offered a Carers Assessment as per Section 75 delegated responsibility from the Local Authority.

You will lead on the completion and inputting of carers assessments, following the process through to payment where identified and keeping a track of those requiring annual reassessments.

An integral part of your role will be to ensure that the role of unpaid carers is recognised, supported and valued, ensuring that carers are well informed, supported with their individual health and well-being needs and involved in decision making about care in relation to their caring role.

You will provide carers with information and advice about services both within and external to the organisation, that contributes to their caring role and increases their knowledge regarding care and treatment processes. You will also signpost and facilitate carers access to a range of support services.

Greater Manchester Mental Health (GMMH) Foundation Trust employs over 6,400 members of staff, who deliver services from more than 160 locations.

We provide inpatient and community-based mental health care for people living in Bolton, the city of Manchester, Salford, Trafford and the borough of Wigan, and a wide range of specialist mental health and substance misuse services across Greater Manchester, the north west of England and beyond.

Greater Manchester is one of the world's most innovative, original and exciting places to live and work. From the beauty of the surrounding countryside to the heart of the vibrant inner city with great shopping, entertainment and dining options.

Wherever you go you will experience a great northern welcome with people famed for their warmth, humour and generosity.

Our people enjoy their work, have opportunities to learn and develop their skills and are encouraged to generate new ideas that improve care for our service users.
