Seven Social Care is looking for a Wellbeing Assessor to work with Stoke on Trent City Council.

Pay rate - £14.94 per hour

To play your part in ensuring that the team is effective and efficient in delivering its core purpose to the citizens of Stoke-on-Trent:
‘Help me to help myself to live my life well.’

The Service provides a range of services to individuals in need of care and support:

- Tailored Information and advice
- Discharge to Assess
- Assessment/Understanding ‘what matters’ to the individual
- Supporting carers
- Building strengths
- Reablement
- Helping people to stay safe from abuse and neglect
The role:
Ensure that people are fully involved and empowered throughout their contact with the service
Ensure that the person is fully understood in order that the most appropriate tailored solutions can be offered.
Identify and address any system barriers which prevent the people from experiencing the most efficient and effective journey through the service.
Ensure that people are empowered, connected, independent and have an improved sense of well-being through the provision of outcome focused support
Meet with people regularly to discuss their experiences of the service to drive further improvement.
