We are looking to recruit a Registered Mental Health Nurse, Occupational Therapist or Social Worker.

We require a highly motivated individual to work with us to improve the transition across services for young people. As well as newly presenting / re presenting 18 - 20 year old’s as part of the Connect18+ service. The post holder will work across the transitions pathway, working primarily with young people 18 years and upwards. This is to ensure that young adults are supported through the pathways between children and adult services. You will work as part of a team in delivering care for younger adults who have a wide range of complex emotional / mental health needs.

There will also be the opportunity to support the CMHS with the depot and clozapine clinics (if RMN) and the advice and support (duty) phone line.

To develop your role you will be supported with an induction package, both local and corporate and you will receive regular supervision and career conversations. You will be closely managed by a senior clinician in the team.

It is widely recognized that the transition point at the age of eighteen is a challenging one for young adults but particularly if they have mental health difficulties. Somerset Foundation Trust is committed to the development of an improved transition pathway, with the aim of ensuring a smooth transition between services as needed.

The post holder will demonstrate a desire to engage in and promote a wide range of therapeutic activities to support service users. Individuals will hold qualities of patience, enthusiasm and a keen interest in working with young people with complex needs.

The post holder must have a full driving licence and have permanent access to own transport with appropriate business insurance.

The post holder will liaise and work in partnership with voluntary agencies, GPs, acute inpatient services, home treatment teams, other community mental health services, primary care, out of hours GP service, Emergency Duty Team, Community Directorate, housing providers, substance misuse services, NHS Direct and the Safeguarding Children Team.
