The Post holder will support delivery of the Healthy Child Programme, and have a key responsibility in undertaking the school health screening and surveillance programme in relation to growth (National Child Measurement Programme).They will play an integral part in supporting the delivery of health promotion, screening, early identification of issues, and making every contact count.

The post holder will work within the 0-19 (25) Integrated Delivery Team to promote the health and wellbeing of school age children so they are able to reach their full potential and not be disadvantaged through ill health.

The post holder will organise and deliver a range of health promotion and prevention activities supporting the wider team to deliver high quality services for families living in the geographical location.

Making Every Contact Count to enable the opportunistic delivery of consistent and concise healthy lifestyle information.

Undertake the growth screening of children as per national guidance for school age screening and NCMP programmes.

Support the 0-19 (25) in the delivery of key school transition sessions.

Promote the needs of children, young people and their families and local communities, working to safeguard children and young people.

Contribute to the development, delivery and reporting of health promoting activities within a variety of settings.

Maintain effective and efficient communication with all statutory, partner and voluntary agencies ensuring the family is at the centre in order to facilitate optimum health and wellbeing for the child, young person and family.

- The post holder will have responsibility for screening activity within a number of schools.
- The post holder is expected to organise and deliver the screening programme within these schools.
- During the screening activity the post holder will be responsible for delivering to the required standard, using MECC principles and ensuring that the data is appropriately recorded.
- The post holder will have responsibility for making referrals within an established pathway and monitoring numbers and type of referral.
- The post holder will work under the direction and supervision of the SCPHN.
- The post holder will support and deliver health promotion activity within community settings.
- The post holder will deliver key areas of the commissioned service specification, national Healthy Child Programme and provide data for indicators within the Public Health Outcomes Framework.
- The post holder will work under the direction and supervision of the Specialist Public Health Practitioner (SCPHN/Staff Nurse/Family Health Nurse).
- Making Every Contact Count to enable the opportunistic delivery of consistent and concise healthy lifestyle information.
- Undertake the NCMP programmes as per Government and Local guidelines.
- Support children through the process and liaise with parents when appropriate and refer on to specialist service in accordance with clinical pathways.
- Enter the data results onto the computer child health system as required.
- Report to Specialist Public Health Practitioner any irregularities detected or observed from screening and surveillance.
- Support the 0-19 (25) in the delivery of key school transition sessions.
- Promote the needs of children, young people and their families and local communities.
- Assist the 0-19 (25) Team in the development of community child health services.
- Promote the needs of children with SEND, young people with SEND and their families in the development of community child health services.
- Promote the needs of children, young people and families from the GRT community in the development of community child health services.
- Deliver a place-based approach with Key Delivery Partners (KDPs) to support the needs of children, young people and families alongside building community resilience.
- Supported by the Community Anchor, practitioners will work within the community to build capacity and increase resilience utilising an asset-based approach.
- Practitioners will need to work in collaboration with KDPs and the Voluntary and Community Sector to map and align services to meet population need, whilst developing innovative ways of working.
- Maintain contemporaneous and accurate records as required by HDFT and in line with professional standards.
