Are you a registered nurse, OT or AHP looking for a change? We are looking for a dynamic, enthusiastic, and motivated individuals with an interest in working in a psychologically and trauma informed way to offer rehabilitation focussed interventions to individuals who have spent long periods of time in hospital. CREST is part of the integrated care systems approach to working differently to improve outcomes for service users, and CREST is the first of these workstreams to become operational.
We are currently expanding as part of our year 2 development during which time we will grow to cover Bradford, Calderdale, Kirklees, Leeds and Wakefield supporting service users reconnect with the community after a stay in complex rehabilitation. The role will involve coordinating the care of individuals in line with changes in national direction of the Care Programme Approach and offering evidence based interventions to address core rehabilitation needs and manage risk within a community setting.
Our commitment to you if successful is that you will be offered strong support structures including reflective practice and clinical supervision, be supported to develop new skills, psychologically informed training and opportunities to be part of evaluation and development of the service.
We are committed to implementing the NHS Workforce Race Equality Standards and welcome staff members from all racial backgrounds who commit to these standards.
CREST support individuals within complex rehabilitation to address barriers to discharge, assess their rehabilitation needs and identify how these could be successfully supported in a least restrictive environment. The varied needs of the service users within CREST’s caseload and their long history of support within inpatient environments mean that CREST need to take an innovative approach to support to ensure successful outcomes.
If successful you will be responsible for a small caseload in line with the care programme approach of service users supported by the wider multi-disciplinary team and on a day to day basis work within the framework of the team approach. You must have good working knowledge of the principles of recovery and practice clinically in a way that promotes therapeutic optimism and hope with service users.
You would also be required to coordinate the delivery of care within the role of duty worker, ensuring the effective prioritisation of care and resources across the shift. There will be the need for close liaison with service users, carers and loved ones, other service providers and the wider MDT to ensure high-quality care is delivered.
Travel will be required across the West Yorkshire Area, with bases in Halifax and Leeds. There will also be some travel to out of area hospitals outside of the West Yorkshire Footprint as part of assessment and engagement.
We are a high quality, high performing NHS foundation trust. We are the main provider of specialist mental health and learning disability services in Leeds. We also provide specialist inpatient services in York as well as some highly specialised services across the country.
We employ around 3,000 staff and every year we have contact with over 25,000 service users. Our vision is to provide outstanding mental health and learning disability services as an employer of choice. Our ambition is to support our service users and carers, our staff and the communities we serve to live healthy and fulfilling lives. We need people like you to help us achieve our goals; to live our lives free from stigma and discrimination; and to improve the lives of people with a learning disability and mental ill health.
We perform well against local and national targets and in our most recent CQC inspection, 85% of our services were rated good or outstanding.
As a teaching trust with strong links to local universities, we are a centre of excellence for teaching, research and development.
CREST was established as part of the complex rehabilitation project with the aim to support repatriation from locked rehabilitation placements and a strong focus on empowering service users and carers in being actively involved their recovery journey. The service currently operates 8am-8pm during the week, and 9-5 over on a weekend with a view to increasing to 8-8 over 7 days.
