As part of your role as a care coordinator, you will be expected to work to a high standard following our policies, care plans, risk assessments and training regime. As a Care Coordinator your daily duties may include but are not limited to (not an exhaustive list).

**JOB ROLE**
- Responsible for carrying out initial assessments, doing reviews and spot checks
- Creating service user care plans, arranging assessments and care plan reviews as required
- Supporting the new and existing care staff team and coordinating the key carers in your designated areas
- Ensuring that all operations are Company and CQC Compliant
- Experience doing rotas and care plans
- Identifying capacity, bidding on and scheduling new care packages with a Can-Do attitude
- Ensure that competent support workers are scheduled to deliver excellent care, safely and consistently.
- Responsible for managing the set-up and coordination of services for New Clients as assigned by care managers
- Manage the ongoing rostering for existing Clients, ensuring high standards of consistency and efficiency are achieved for clients with varying requirements
- Participate in an emergency on-call rota to provide out of hours support to clients and staff. Covering emergency care calls in case of carer's sick or absent
- Working with recruitment team for hiring the best resources

**YOU WILL NEED TO HAVE/BE**
- You will ideally hold an NVQ level 3 (or equivalent) in Health & Social Care or a related topic or be willing to study towards this.
- Excellent communication and customer service skills are key to this role
- A good understanding of healthcare and a desire to help people live a happier, fuller life
- You must have a full valid Driving Licence with access to a vehicle
- Good organisational and communication skills to ensure an excellent service
